If you are in an Academic Clinical Fellow post or a Clinical Lecturer post (NIHR and locally funded) then you will need to complete the following document (Annex A) and submit to your school support manager in advance of your assessment with all your other paperwork. You should have a meeting with your academic supervisor at least one month prior to your assessment in order to complete this. It is advisable to meet two months in advance to discuss the paperwork, and then one month in advance to sign in off for submission for your assessment.
