College Notification Form

This form should be completed in full prior to submission to the College. You should return the completed form to the College with a cheque for the appropriate amount (£150 for single CCT or £195 for dual CCT) and copies of your RITA forms including the RITA Form G: Final Record of Satisfactory Progress. Although it is not a College requirement that you enclose a copy of your CV with the form, it is often helpful, especially if you have undertaken a prolonged or unusual training programme.

If you have had any of the following approved by the College towards your CCT you must enclose copies of the relevant approval letters with your form:

  • post-MRCPsych registrar training
  • research
  • flexible training
  • locum SpR/SR training exceeding 3 months
  • training overseas
If you have obtained a substantive consultant post, you should attach a covering letter stating this and including details of the employing Health Board, the specialty and the start date.

If you have any queries, please contact Lee Jordan, Specialty Training Administrator, Department of Professional Standards, Royal College of Psychiatrists, 17 Belgrave Square, London SW1X 8PG. Telephone number 020 7235 2351 ext. 270; fax 020 7235 7976; email Lee Jordan. This is the address to which completed College Notification Forms and other required documents should be sent.
